There's an interesting article in the NY Times about North Korea seeking to increase tourism by introducing cruise ships. Looks like they've got a ways to go :-). Europeans will be encouraged to sign up in October. North Korean holiday cruises

Koryo is the main tour operator. They also have an add-on to visit abandoned, former heavy industrial sites of the Soviet Union, which sounds really interesting!
Attended the annual travel fair in Helsinki this month and there was a tour operator that claimed to have a right to arrange individual tours (with a local guide) starting from China side. If interested, I can refer you to this operator that also sells TransSiberian railway tours to Ulan Bator, Beijing and Vladivostok.
